1. Prepare for your visit
At Sarpy County OB-GYN, P.C. we understand the importance of comfort and the value of time, which is why we strive to accomodate to your needs.We recommend you arrive 10-15 minutes prior to your scheduled appointment to fill out necessary patient information. Remember to bring your insurance card, referral and co-payment, if required by your plan.
Try to write down any specific questions you may have to ensure all of your concerns are addressed. We hope your first visit with Sarpy County OB-GYN, P.C. is a pleasant one.
2. Download forms
Please fill out all necessary forms and bring them with you.• Consent to Outpatient Service
• Patient Intake History
• Protected Health Information (PHI)
• Receipt of Notice of Privacy Practices
3. What to bring
• Your insurance card. If you have several medical insurances, you need to bring all their cards with you and notify our front desk staff about all of them.
• A list of all medications you are currently taking. Include the dosage and frequency found on the Rx label. Please list non-prescription medicines and supplements as well.
• The name and phone number of your primary care doctor(s). If you have not already arranged for your medical records to be transferred, please do so as soon as possible.
• Completed forms downloaded and printed from the "Download forms" list above.
4. Billing and Payment
